Abstract
Purpose : Authors accessed the clinical feasibility of 3D CT reconstruction in the imaging of thoracolumbar fracture by comparing with MRI.

Materials and Methods : Back injured 17 patients who underwent MRI and CT simultaneously were enrolled. On 3D sagittal reconstruction CT images and sagittal T2 MR images, the degree of compression, burst, facet joint involvement, posterior ligament injury, posterior arc injury, and cord injury were correlated and compared.

Results : All bony injuries such as compression (17 cases), burst (10 cases), facet involement (2 cases), posterior arc fracture (7 cases) were well demonstrated on 3D reconstruction compared to MRI. but ligament (6 cases) and cord injury (6 cases) were not and doubtful.

Conclusion : 3D reconstruction CT image could be useful in initial or screening tool of thoracolumbar fractures but in suspicion of soft tissue injury, MRI is mandatory.
