Effect of Intradiscal Steroids in Ruptured Type Patients without Pain Provocation by Discography

Abstract
Objective: To determine the therapeutic effect of intradiscal steroid injections in the ruptured type patients without pain provocation by discography.

Methods: Eighty discogenic low back pain patients confirmed ruptured type in discography were studied. Pain response was recorded as: no pain, dissimilar, similar, or exact. Fourteen intradiscal steroids injection using 40 mg of triamcinolone and 1.0 cc of 2% lidocaine was performed in patients without pain provocation. Effect was evaluated for visual analogue scale (VAS) and limitation of lumbar flexion between pre-injection and post-injection.

Results: Pain response in ruptured type patients was shown as follows: no pain, 58 (72.5%); dissimilar, 6 (7.5%); similar, 7 (8.6%); exact, 9 (11.3%). After steroid injection, the averages of VAS reduced significantly (p£¼0.05). Subjects of severely or moderately limited lumbar flexion was decreased from 10 at pre-injection to 5 at 1 week post- injection. Twelve out of 14 patients (85.7%) had a successful long-term outcome, reporting at least an reduction over 50% in VAS 3 months post-injection.

Conclusion: Existence rate of pain provocation is low in the ruptured type, because provocation may be not reacted by low intradiscal pressure in ruptured disc. And intradiscal steroids are helpful in ruptured type patients with no pain provocation.
