Short-term Effect of Sympathetic Block on Lymphedema in Lower Limbs-A Pilot Study-

Objective: To evaluate the short-term effect of lumbar sympathetic block on lymphedema in lower limbs. Materials and
Methods: Seven patients with chronic lymphedema in the lower limbs confirmed by lymphoscintigraphy were enrolled. Unilateral lumbar sympathetic chain block with the mixture of 4 ml of 1% lidocaine and 1 ml of 40 mg triamcinolone was performed under fluoroscopic guide. The circumferences of both lower limbs were measured at pre and post-intervention.
Results: The limb circumferences of injected sides decreased significantly after intervention at ankle, lower leg (10 cm below knee), distal thigh (10 cm above knee), and proximal thigh (20 cm above knee) (p£¼0.05). Mean reductions were 13.0¡¾11.6 mm at ankle, 14.7¡¾9.1 mm at lower leg, 4.7¡¾9.0 mm at knee, 21.3¡¾26.1 mm at distal thigh, and 16.6¡¾14.8 mm at proximal thigh. The limb circumferences of contralateral sides also decreased after the intervention at knee, distal thigh, proximal thigh (p£¼0.05).
Conclusion: Lumbar sympathetic block reduced the leg swellings of the patients with chronic lymphedema in the lower limbs. It was effective on reduction of both proximal and distal parts of swollen legs. Further studies are needed to confirm the effect of lumbar sympathetic block on lymphedema in lower limbs.
