A 73-year old woman with a history of preceding influenza vaccination was admitted complaining of abrupt onset bilateral weakness of ankle and toes muscles and severe pain and paresthesia in both lower legs. Guillain-Barre syndrome was diagnosed by electrodiagnostic findings and typical albumin-cytologic dissociation of cerebrospinal fluid analysis, although her symptom and sign were far different from the usual presentation in typical post-infectious Guillain-Barre syndrome. Early treatment including a series of intravenous injection of human immunoglobulin and anticonvulsant medications helped to recover her motor symptom as well as sensory dysfunction. On 21st day after the onset of the disease, her symptom improved enough to be discharged walking independently with gait aids. Our case reminds us the possible relationship of influenza vaccination and the development of atypical type of Guillain-Barre syndrome.
