Intramuscular type represents the most common lesion of deep soft-tissue hemangioma, although they are themselves still quite rare. Hemangiomas occur in 1¡4% of the general population. Intramuscular hemangiomas represent £¼1% of all hemangiomas and consist of benign proliferation of blood vessels. Typical presentation is an enlarging painful soft-tissue mass without cutaneous changes. In this case, a 30-year-old female visited with a swelling in the right calf region associated with pain which was aggravated on walking since 5-year-old without history of preceding trauma. The plain film showed the abnormality of soft tissue enhancement and intramuscular fatty streaks. Ultrasound revealed a hypoechoic irregular mass located in the gastrocnemius muscle. Color Doppler signal was not increased. The MRI findings for these lesions consist of intermediate signal intensity on T1 weighted image and a strikingly intense signal on the T2-weighted image. All these features were favoring the diagnosis of huge intramuscular cavernous hemangioma.
