Abstract
Today¡¯s implantable cardioverter-defibrillators (ICDs) are sophisticated systems that are equipped with a variety of features and algorithms that discriminate between the rhythms that the device will treat and those it will ignore. As ICD systems have evolved and the indications for their implantation have expanded, clinicians are now faced with decisions that go beyond the application of the technology to distinguish between ventricular and supraventricular arrhythmias, as they must now consider questions of a more philosophical nature, such as when therapy is truly necessary. Future technological developments are likely, and while they may provide users with additional tools, they may also raise new questions about their appropriate applications.
