Antiplatelet agents have been used conventionally in patients with cardiovascular diseases. Dual antiplatelet therapy (DAPT) is a combination therapy of aspirin and P2Y12 inhibitors, such as clopidogrel, prasugrel, and ticagrelor, and is used to prevent thrombosis in patients with coronary artery diseases. Nevertheless, the risk of perioperative bleeding during invasive dental procedures in patients receiving dual antiplatelet agents is relatively high. Discontinuation of this therapy is not recommended unless the risk of bleeding is very high. Several studies have reported discontinuing drugs during tooth extraction or dental implant placement in patients on DAPT. On the other hand, few studies have reported on immediate implant placement and maxillary sinus graft in patients on DAPT. This paper reports a case of immediate implant placement and maxillary sinus graft in a 48-year-old patient without discontinuing the dual antiplatelet agents, who previously had undergone percutaneous coronary intervention (PCI) for unstable angina.
