There are various facial pain syndromes including trigeminal neuralgia, sphenopalatine neuralgia and persistent idiopathic facial pain syndrome (PIFP). The blockade of sphenopalatine ganglion (SPG) has been proven to be effective in pain caused by aforementioned pain syndromes and has been used to treat them. Both post-ganglionic sympathetic neurons and somatic sensory afferent branches of the maxillary division of the trigeminal nerve pass through this ganglion. PIFP may frequently relapse with a sympathetic component, which makes neurolysis of the SPG rather than single blockade of the stellate ganglion block more ideal. Herein, we introduce a case of a 35-year-old male with severe facial pain in left maxillary region, whose diagnosis was persistent idiopathic facial pain syndrome. We report a unique case of the patient whose PIFP was completely relieved after a single SPG block without any recurrence.
