The most classical and common treatment for hydrocephalus is cerebrospinal fluid (CSF) shunting. However, the shunt is effective in treating hydrocephalus, while the many and various complications cannot be ignored. We present 1 case of rare non-traumatic shunt complication in a 7-year-old girl. Imaging studies showed the intact shunt catheter and valve, and fluid collection around the shunt valve. But, in the operation field, we confirmed no abnormality in the shunt catheter, but we found a small separation between the base and housing of the shunt valve. The authors suggest again that if there is CSF leakage, the neurosurgeon should have a revision operation even if the shunt system appears to be intact on the imaging studies.
