Adult scoliosis often presents with the symptoms of root compression unilaterally or bilaterally. Microscopic bilateral decompression via unilateral approach in degenerative lumbar stenosis or scoliosis is a good surgical option. But in scoliotic patient with symptoms of unilateral stenosis, the direct approach to the symptomatic side is sometimes not feasible, if the stenotic side has the concavity, due to secondary rotation of vertebral bodies to the concave side. We present a minimal invasive, in terms of facet preservation, and useful technique of convex-side approach for contralateral concave side decompression in degenerative lumbar scoliosis, which is a very easy procedure and its result is successful.
