Comparison of Surgical Site Infection between Laparoscopic Appendectomy and Open Appendectomy

Abstract

|
Objectives: Laparoscopic appendectomy has gradually become a routine procedure. The role of laparoscopy, however, in the setting of perforated appendicitis remains controversial. And surgical site infection after appendectomy is relative common complication. A retrospective study was conducted to evaluate the early postoperative outcomes of laparoscopic appendectomy compared to open appendectomy in patients with acute appendicitis.
Methods: A total of 824 patients required an appendectomy due to acute appendicitis between January 2011 and December
2012. Among these patients, 694 were older than 15 years. Operative times, length of hospital stay, overall complication rates within 30 days, and surgical site infection (SSI) rates were analyzed.
Results: Out of the 694 patients, 547 required laparoscopic appendectomy and 147 open appendectomy. Age, gender, and
history of diabetes were similar in two groups. Operative time was longer in laparoscopic appendectomy than open appendectomy, but length of hospital stay was shorter in laparoscopic appendectomy. Perforated appendicitis required open appendectomy more than laparoscopic appendectomy (p<0.001). SSI rates including organ/space SSI rates were similar in two groups in simple appendicitis and perforated appendicitis, each.
Conclusion: Laparoscopic appendectomy is a safe and feasible procedure in perforated appendicitis as well as simple appendicitis.
