The unexpected pandemic of Coronavirus disease 2019 (COVID-19) has impacted many lives and affects the whole healthcare systems globally. We established two negative pressure operating rooms, and healthcare workers were equipped with personal protective equipment according to the institutional COVID-19 control guidelines. We share the infection prevention strategies for the elective and emergent surgical procedures at our hospital. A 54-year-old man who was COVID-19 confirmed underwent emergent laparoscopic appendectomy and treated well without adverse events or perioperative COVID-19 transmission. Setting up the negative pressure operation room and proper application of personal protective equipment are mandatory to perform safe and sustainable operation during COVID-19 pandemic. Our approach with setting up negative-pressure operation rooms and strategies based on well-organized system, experienced staffs, active team monitoring, and competency of team communication can be referred in cases of elective and emergent operations during COVID-19 pandemic.
