A 70-year-old Korean woman was diagnosed as early gastric cancer of moderately differentiated type located in the upper third of the stomach and received cardia-preserving proximal gastrectomy in 2007. Postoperative follow-up esophagogastroduodenoscopy was done twice a year. Three years after initial operation, a new gastric remnant cancer was found 4 cm below the anastomotic line. Completion total gastrectomy was performed and the biopsy result was adenocarcinoma of signet ring cell type. The reported patient was a very rare case of recurrent cancer detected early after proximal gastrectomy.
