To make an appropriate treatment plan, it is necessary to accurately diagnose the patient. Especially, it is necessary to identify the occlusion relation that is the basis for the establishment of the treatment plan, and if the patient¡¯s occlusion is unstable and the reproducibility of the recording date decreases, it is necessary to first establish the stability of the occlusion including the position of the mandibular condyle and then make a diagnosis. This case report introduces a nonsurgical treatment with strategic unilateral extraction in a skeletal Class II patient with unstable occlusion and facial asymmetry with a 7-year follow-up. Since the patient showed unstable occlusion in the initial visit, re-diagnosis was done after using the stabilization splint. Considering facial asymmetry and severe Class II molar relation, the left mandibular premolar was not extracted. The normal molar relation was improved with stable occlusion and harmonious facial profile. The treatment result was stable at a 7-year follow-up.
