One-Jaw Surgical Orthodontic Treatment of Class III Malocclusion with Facial Asymmetry Using Microimplant

Abstract
For the correction of facial asymmetry in adult patients, clinicians usually consider two-jaw surgery. However, facial asymmetry can be effectively corrected with mandibular surgery alone by using microimplants to improve maxillary canting. A 20-year-old man visited our clinic with complaints of facial asymmetry and mandibular protrusion. He exhibited severe menton deviation, anterior openbite, anterior crossbite, and occlusal canting. Due to skeletal discrepancies, his anterior and posterior teeth were compensated. In preparation for orthognathic surgery, decompensation was carried out using microimplants. After 15 months of presurgical orthodontic treatment, decompensation was successfully completed. Bilateral sagittal split ramus osteotomy (BSSRO) was performed to correct the skeletal discrepancies. Following the surgery, intermaxillary elastics were utilized on the microimplants to correct any surgical errors. After 25 months, the treatment was concluded, resulting in facial symmetry and good occlusion.
