Abstract
Purpose: To demonstrate cases with bialveolar protrusion treated by the customized lingual orthodontic system, thereby to review the basic biomechanical principles of the system, and to discuss the pros and cons related to its clinical application.

Methods: The innovative digital technology has landed in the orthodontic avenue, and its implementation results in inventive changes in current diagnosis and treatment. The novel image technologies such as laser scanning, structure photo-imaging, surface image analysis have driven out plaster model from the clinical environment. The customized lingual orthodontic system herein adopts virtual 3D image processing technology, which secures predicted and planned results by providing individualized lingual brackets and wires. The individual appliances are produced by robotics which fit with anatomic diversity of each patient, and consequently bring us detailed final tooth position. The system substantially reflects the mechanical requirements for individualized treatment especially for lingual orthodontics. The primary advantage of the biomechanics involves feasibility to make the lingual orthodontics for those who are already familiar with any other lingual mechanics as well as for the novices who have done traditional labial techniques.

Results: Fully customized lingual brackets is effective treatment option to treat bialveolar protrusion cases.

Conclusion: The demand for invisible esthetic orthodontics is expected to further increase, and orthodontists should investigate these therapeutic options in order to satisfy patient demand. Invisible orthodontic devices such as the Incognito provide brackets customized to fit individual teeth and a wire with complex bending properties using robotics. Therefore, this treatment option is recommended for orthodontists experiencing difficulty with lingual correction or performing lingual correction for the first time.
