Lingual Orthodontic Treatment Using Twin-wire Technique in Extraction Cases

Abstract
Purpose: Th e most prevalent diffi culty found in lingual orthodontics involving extraction is a loss of torque control in anterior region. Th is problem is almost inevitable in lingual orthodontics especially in maxillary anteriors due to its force mechanism causing malocclusion, severe overbite, severe overjet, misalignment of incisal edges and etc.

Results: To resolve this problem in lingual orthodontics, clinicians have employed a variety of methods such as placing excessive torque during set-up, applying additional torque while bracket bonding, utilizing segmental technic, loop mechanic, lever arm, and etc. Such methods were found to be technic sensitive, governed heavily by host factors, and diffi cult to control which may lead to unfavorable clinical results. Also, additional torque may not dissipate in timely manner which may act as another hindrance in achieving adequate occlusion, overbite and overjet. A novel method called Twin archwire techinic is hereby introduced which is less technic sensitive utilizing conventional multi-slot lingual bracket for proper torque control in maxillary anteriors.

Conclusion: Twin archwire technic is a less technic-sensitive and user-friendly method in alleviating anterior torque control problem in lingual orthodontics which requires conventional ideal set-up with multi-slot lingual brackets without additional wire bending and set-up.
