Let TADs Talk!: Non-surgical Orthodontic Treatment of Skeletal Anterior Openbite Using TADs with Fully-customized Lingual System

Abstract
The early summer of 2017 marked the unforgettable event of the 7th World Society of Lingual Orthodontics (WSLO) Annual Meeting. Hundreds of international delegates across different continents went on their pilgrimage to Bangkok not just for her exceptional hospitality and cuisine, it¡¯s a hand-in-hand biennial rendezvous among all enthusiastic and passionate ¡®Lingual Orthodontists¡¯. I was very honored to present my recent clinical studies with using fullycustomized lingual orthodontic system (named ¡°Let TADs Talk!¡±) on the first day of the 2-day-long conference. Anchorage control is one of the biggest challenges for the successful orthodontic treatment. Until the recent decade, the advancement of skeletal anchorage has expanded the boundaries of orthodontic treatment. Temporary anchorage devices (TADs) have been advocated in providing absolute anchorage for orthodontic treatment predictably. Anterior open bite malocclusion is considered to be one of the most difficult orthodontic problems since its etiological factors are complicated and multifactorial. Treatments include modification of functional habits, orthopedic treatment, orthognathic surgery, and orthodontic treatment with extrusion of the anterior teeth or intrusion of the posterior teeth. Traditional treatment options are mostly limited in orthognathic surgery. The risk and cost of the surgery would however, often discourage people from considering surgical treatment. The lecture aimed at demonstrating the versatility of TADs application in challenging orthodontic cases of skeletal anterior open bite in lingual orthodontic system. Careful case diagnosis, treatment planning and the potential complications related to TADs clinical use and long-term retention protocols were discussed. The objectives of the lecture were to allow participants to understand the current concept and therapy on using TADs for vertical control; recognize the case diagnosis, treatment alternatives and limitations; design TADs insertion sites & respective biomechanics in lingual system; and to be familiar with the complications of TADs application.
