Surgical Outcome of Pars Plana Vitrectomy Performed by Vitreoretinal Fellows

Abstract
|
|
|
Purpose: To evaluate the surgical outcome of pars plana vitrectomy performed by vitreoretinal Fellows.
Methods: Clinical records of patients who received pars plana vitrectomy by vitreoretinal Fellows with less than two years of experience were retrospectively reviewed. Number of surgeries per each surgeon, patient characteristics, surgical indications, number and reasons for reoperation, and intraoperative complications were evaluated.
Results: Two hundred twenty-five eyes of 235 patients were included in this study. The average number of surgeries per year per surgeon was 11.2 ¡¾ 2.4 (range 0-34), and number of surgeries performed during the 2nd year of fellowship training was significantly higher than that in the 1st year of fellowship training (17.2 ¡¾ 11.9 vs. 5.7 ¡¾ 6.99, p = 0.013). Common surgical indications were as follows: proliferative diabetic retinopathy with vitreous hemorrhage (22.1%), rhegmatogenous retinal detachment (19.6%), epiretinal membrane (18.3%), crystalline lens drop, or remnant lens associated with cataract surgery (9.4%). Nearly half of patients showed improvement of ¡Ã2 lines in visual acuity, and the rate was higher in 2nd year Fellows than in 1st year Fellows, or as the number of cumulative surgeries increased. Thirty-one eyes needed reoperation and that rate was decreased for 2nd year Fellows. Rates of posterior capsular rupture in combined phacovitrectomy was significantly higher than simple phacoemulsification cataract surgery (21.8% vs. 12.0%, p = 0.032) in vitreoretinal Fellows.
Conclusions: Vitreoretinal Fellows performed pars plana vitrectomy with various indications, and the surgical results improved as experience accumulated.
