Abstract
Purpose: To analyze the clinical characteristics and treatment prognosis of bilateral posterior scleritis in Koreans.

Methods: As a retrospective cohort study, demographics, clinical characteristics treatment methods and final visual acuity were analyzed in patients diagnosed with bilateral posterior scleritis in a single tertiary institution.

Results: Six patients with bilateral posterior scleritis were enrolled in the study, including five women (83.3%), and the average age was 60 years. The main symptom was headache in four patients (67%), and optic disc edema was found in 83.3%. Systemic steroid treatment was performed in all patients. Recurrence, observed in 66.7% of cases, was effectively managed through steroid treatment. It was confirmed that good initial visual acuity was associated with better final vision (R2 = 0.382, p = 0.032).

Conclusions: Bilateral posterior scleritis is a rare scleral inflammatory disease for which a good vision prognosis can be obtained through active systemic steroid treatment with early diagnosis.
