Clinical value of chest computed tomography in the detection and management of pulmonary metastasis for colorectal cancer patients who have undergone curative resection

Abstract
Purpose : To evaluate the clinical value of chest CT compared to chest X-ray and abdominopelvic CT in diagnosis and management of pulmonary metastasis for colorectal cancer patients underwent curative resection.

Materials and methods : The records of 247 patients with pulmonary metastasis confirmed by chest CT after curative surgery were reviewed. On the basis of chest CT findings, the detection and the accuracy rates for diagnosis of patterns of pulmonary metastasis [single (unilateral single) vs. multiple (unilateral > 2 or bilateral)] of chest X-ray and abdominopelvic CT were compared. The management methods and oncologic results according to the pattern were analyzed.

Results : The detection and the accuracy rates for diagnosis of the pattern of metastasis of chest X-ray were 64.4% and 49.0%, respectively. Those of abdominopelvic CT were 54.3% and 37.9%, respectively. The rates of surgical resection for pulmonary metastasis and 3-year oveall survival rates were different according to the pattern of metastasis (resection : single 74.8%, multiple 14.2%, p<0.001, 3-year OS : single 67.9%, multiple 27.1 %, p<0.001).

Conclusion : Chest CT could provide more accurate information on pulmonary metastasis, and it could affect the management methods and the prognosis. A follow-up program including chest CT should be considered after curative resection in colorectal cancer patients.
