Abstract
To report the fatal outcome of hemoperitoneum caused by spontaneous utero-ovarian vein rupture during pregnancy in a patient with endometriosis (ES). A 33-year-old patient with a prior history of endometriosis developed spontaneous left utero-ovarian vein rupture accompanied by fetal death in utero at 32 weeks gestation. Despite surviving hypovolemic shock, systemic inflammatory response syndrome due to hypoxia-reperfusion injury in multiple organs eventually led to maternal death. Obstetricians must be aware of the possibility of spontaneous utero-ovarian vein rupture, especially if a pregnant woman with endometriosis presents with symptoms of hemoperitoneum.
