Clinical significance of the preoperative carcinoembryonic antigen level in primary epithelial ovarian cancer

Abstract
Objective: This study was designed to evaluate the clinical significance of preoperative serum carcinoembryonic antigen (CEA) as a prognostic marker in epithelial ovarian cancer.

Method: We performed a retrospective review of the medical records of one hundred eleven patients with epithelial ovarian cancer who underwent surgery at Seoul National University Bundang Hospital between June 2003 and March 2009. The correlations of CEA with other tumor markers (CA-125, CA 19-9, and CEA) and clinicopathological prognostic factors (tumor stage, age, residual mass, peritoneal washing cytology and preoperative tumor size) were analyzed.

Results: The normal cutoff levels of CEA, CA-125, and CA 19-9 were 5 ng/mL, 35 U/mL, and 37 U/mL, respectively. Of the one hundred eleven patients, only 8.1% of the patients were positive for CEA, and all patients with an elevated CEA level had a high CA-125 level. The sensitivity of CA-125 was 83.6%, and that of CA 19-9 was 21.9%. Correlation analysis between tumor markers and prognostic factors showed that CA-125 correlated significantly with advanced tumor stage, positive washing cytology and presence of a residual mass. CA 19-9 was significantly associated with advanced tumor stage, positive washing cytology, the histological type and preoperative tumor size. However, CEA had no significant correlation with all prognostic factors except the mucinous histologic type.

Conclusion: The results of this study suggest that serum CEA may have a limited role in detecting and predicting the prognosis of patients with ovarian cancer. Thus, the preoperative evaluation of serum CEA level may have a limited value in the prognosis of ovarian cancer.
