Abstract
A congenital cystic adenomatoid malformation (CCAM) is a relatively rare developmental abnormality of the lung. Because of advances in antenatal sonography, CCAMs are increasingly diagnosed prenatally. The prognosis of CCAMs is variable, ranging from spontaneous in utero regression to perinatal death, and the optimal treatment modality for CCAMs based on size and type is not well-established yet. We managed a case of a large CCAM with a prominent mediastinal deviation. With serial sonographic evaluations, we managed the patient expectantly. At birth, the baby had no respiratory distress and had stable vital signs. Now the baby is 18 months old; a follow-up chest x-ray showed that the lung lesion had resolved. This case report shows the importance of conservative management with serial images when a CCAM is detected, and the possibility of spontaneous regression of a large CCAM lesion.
