Diagnostic Capability of 3-Dimensional Optic Nerve Head Analysis Using Stereo Retina Camera in Glaucoma

Abstract
Purpose : To evaluate the diagnostic capability of quantified optic nerve head (ONH) depth analysis system incorporated in the Nonmyd-WX 3D retina camera (Kowa, Tokyo, Japan) in discriminating between glaucoma suspects and glaucoma subjects.

Methods : The ONHs of 47 glaucoma suspects and 67 glaucoma subjects underwent 3-dimensional ONH depth analysis using Nonmyd-WX. Glaucoma diagnostic ability of various ONH topographic parameters were analysed by assessing the area under receiver-operating characteristic curves (AUROCs) and the sensitivities at fixed specificity. Correlation and agreement with spectral domain optical coherence tomography (SD-OCT; Cirrus OCT, Carl Zeiss Meditec, Dublin, CA) for each ONH topographic parameter were evaluated by using the linear regression and the Bland-Altman plot, respectively.

Results : All ONH measurements were significantly different between the glaucoma subjects and glaucoma suspects. Among all the parameters examined, superior rim width showed the largest AUROC (0.789) with the sensitivity of 73.13% at specificity of 80%. Nonmyd-WX tended to define the disc area and rim area as larger, and the vertical cup-to-disc ratio, cup-to-disc area ratio and cup volume as smaller than Cirrus OCT. Significant proportional biases were observed between Nonmyd-WX and Cirrus OCT for rim area, cup-to-disc area ratio and cup volume.

Conclusions : Topographic ONH depth analysis of stereoscopic image pair using Nonmyd-WX showed moderate glaucoma diagnostic capability in glaucoma subjects and glaucoma suspects. The clinical usefulness remains to be determined by a larger scale study including healthy controls.
