Diagnostic Ability and Reproducibility of Retinal Nerve Fiber Layer Thickness Measurement Based on Bruch's Membrane Opening

Abstract
Purpose : To analyze the reproducibility of retinal nerve fiber layer (RNFL) measurement with Spectralis optical coherence tomography (OCT) in normal subjects and glaucoma patients and compare the diagnostic ability between Spectralis OCT and Cirrus OCT.

Methods : Spectralis OCT and Cirrus OCT were used to measure RNFL thickness of 58 healthy eyes and 62 glaucoma eyes. During Spectralis OCT, the RNFL thickness around the 3 peripapillary circle scans from the center of the Bruch's membrane opening was measured. Intraclass correlation coefficient (ICC), Coefficient of variation (CV), and intravisit repeatability were analyzed. The diagnostic ability was compared through area under receiver operating characteristic.

Results : Reproducibility of RNFL thickness measurement with Spectralis OCT according to 3 peripapillary circle scans is ICC 0.996/0.991/0.986, CV 1.30/2.04/2.09%, and intravisit repeatability 2.80/3.79/3.02 ¥ìm, all three were the best at 3.5 mm. In Spectralis OCT, glaucoma diagnostic ability was high in the order of 4.1 mm, 4.7 mm, and 3.5 mm, but there was no statistically significant difference. When comparing the diagnostic ability of 3.5 mm Spectralis OCT and Cirrus OCT, there was no statistically significant difference (p=0.461).

Conclusions : The Spectralis OCT showed excellent reproducibility and diagnostic ability in retinal nerve fiber layer thickness measurement, and the diagnostic ability was not significantly different from that of Cirrus OCT.
