Optic Nerve Head Drusen with Structural and Functional Loss

Abstract
Purpose : To report a case of optic nerve head drusen with macular ganglion cell/inner plexiform layer (GC/IPL) and circumpapillary retinal nerve fiber layer (RNFL) loss and visual field defect.

Case summary : A 21-year old female visited our clinic for further evaluation of the RFNL defect found at local clinic. The best corrected visual acuity was 1.0 in both eyes and intraocular pressure was 15 mmHg in right eye and 14 mmHg in left eye with Goldmann applanation tonometry. Dilated fundus examination showed bilateral elevated optic disc with blurred disc margin and RNFL defect in both eyes. Optical coherence tomography showed ovoid mass-like structure with signal-poor core and hyperreflective margin and inferior macular GC/IPL and circumpapillary RNFL damage in both eyes. Automated perimetry showed superior nasal step corresponding to the structural damage in both eyes.

Conclusion : Circumpapillary RNFL and macular GC/IPL damage with corresponding visual field defect that mimic glaucoma may occur in a patient with optic nerve head drusen.
