The Measurement of Diurnal Intraocular Pressure and Blood Pressure in Medically Controlled Glaucoma with Progression: Descriptive Study

Abstract

|
Purpose : To investigate the percentage of abnormal diurnal intraocular pressure (IOP) and blood pressure (BP) in medically controlled glaucoma with progression.
Methods : We retrospectively reviewed the medical charts of 88 patients with glaucoma who were admitted to our glaucoma clinic for diurnal IOP check. Mean deviation of visual field test, type of glaucoma, number of IOP lowering medication, diurnal IOP and BP data were obtained. We also investigated the reasons for diurnal IOP measurement and change of treatment plan afterwards. We defined the IOP peak when IOP was 22 mmHg or more. Systolic blood pressure (SBP) dip and diastolic blood pressure (DBP) dip were defined as SBP <100 mmHg and DBP <60 mmHg, respectively. Mean ocular perfusion pressure (MOPP) dip was defined when MOPP was less than 40 mmHg.
Results : Of the total 88 patients, 51 were male and 37 were female, and the mean age was 55.1 ¡¾ 16.2 years. The average number of glaucoma medications used was 2.8 ¡¾ 1.3. The mean IOP measured at daytime before admission was 14.6 ¡¾ 3.5 mmHg. Among entire patients, IOP peak occurred more than once in 18 patients (20.5%), SBP dip in 41 patients (46.6%), DBP dip in 42 patients (47.7%) and MOPP dip in 66 patients (75%). After diurnal IOP measurement, 49 patients (55.7%) changed their treatment method to add medications, laser, or filtering surgery.
Conclusions : Nocturnal IOP spikes and hypotension were found during measurement of diurnal IOP and BP in medically controlled glaucoma with progression, and about 50% of the patients changed their treatment plan.
