The Effect of Epiretinal Membrane Peeling in Open Angle Glaucoma Patients with Epiretinal Membrane

Abstract
Purpose : To investigate the effect of epiretinal membrane removal surgery on the progression of glaucoma after surgery in patients with open-angle glaucoma and epiretinal membrane.

Methods : A longitudinal and retrospective study was conducted on 37 patients (37 eyes) who had both open-angle glaucoma and epiretinal membrane. Group 1 was a group that had both epiretinal membrane peeling and cataract surgery, and group 2 was a group that only had cataract surgery. Parameters of visual field test (VFT) such as visual field index (VFI), mean deviation (MD), pattern standard deviation (PSD) were measured and macular ganglion cell-inner plexiform layer (GCIPLT), and retinal nerve fiber layer thickness (RNFLT) of optical coherence tomography exam were evaluated before and 1, 2, and 3 years after operation.

Results : In group 1, VFI decreased from 85.94% preoperatively to 78.06%, 73.41%, and 69.41% postoperatively in group 1, while increased from 74.40% preoperatively to 77.00%, 76.80%, and 78.90% postoperatively in group 2. The MD deteriorated from -7.17 dB preoperatively to -9.33 dB,-10.66 dB, and -11.42 dB postoperatively in group 1, whereas the MD improved from -10.58 dB preoperatively to -9.76 dB, -9.34 dB, and -8.22 dB postoperatively in group 2. In repeated measured-analysis of variance (RM-ANOVA), the differences between the two groups according to time were significant for VFI, MD, and PSD of the VFT (p < 0.001). GCIPLT decreased from 63.71 ¥ìm preoperatively to 53.12, 44.24, and 41.59 ¥ìm postoperatively in group 1, but increased from 74.45 ¥ìm preoperatively to 79.70, 78.35, and 82.45 ¥ìm postoperatively in group 2. RNFLT decreased from 83.59 ¥ìm preoperatively to 79.41, 73.65, and 65.47 ¥ìm postoperatively in group 1, but increased from 81.90 ¥ìm preoperatively to 82.70, 82.15, and 84.75 ¥ìm postoperatively in group 2. GCIPLT and RNFLT had the difference between the two groups according to time statistically significantly in RM-ANOVA (p < 0.001).

Conclusions : In patients with open-angle glaucoma and epiretinal membrane, if epiretinal membrane removal is performed, attention should be paid to the prognosis after surgery, keeping in mind that the progression of glaucoma can be aggravated.
