Glaucoma Surgery and Subsequent Retinal Vessel Occlusion Incidence in Elderly Patients: A Population-based Preliminary Study

Abstract
Purpose : To determine the post-glaucoma-surgery incidence and comparative risk of retinal vessel occlusion (RVO) in a nationwide elderly-glaucoma population sample cohort.

Methods : This retrospective, observational, nested case-control study included patients with glaucoma who were RVO-free at the baseline (n = 50,808) in the 2017-2018 Aged-Patient Sample cohort (n = 1,409,624; 10% extracted from the total Korean population over the age of 65) provided by Korean Health Insurance Review and Assessment Service. Patients who were newly diagnosed with RVO (n = 1,498) were 1:2 matched (using a propensity-score matching method) to 2,996 controls by age, sex, and Charlson Comorbidity Index. Subsequent RVO incidence and adjusted odds ratio (OR) of incident RVO following glaucoma surgery were identified.

Results : Subsequent RVO incidence was 12.5% (95% confidence interval [CI], 7.1-17.9%) in the elderly patients who had undergone glaucoma surgery. The adjusted OR of incident RVO following glaucoma surgery was 6.99 (95% CI, 2.06-23.73).

Conclusions : We found that glaucoma surgery is significantly associated with increased RVO risk among elderly glaucoma patients. Verification and risk factor analysis studies using the nationwide population-based cohort or the UK Biobank database are needed.
