Immediate re-implantation after implant removal is ideal in terms of reduced treatment duration. However, in a case of severe peri-implantitis, it is difficult to predict three-dimensional changes after explantation. Alveolar ridge preservation (ARP) is an effective approach to avoid enormous soft and hard tissue augmentation. It can also reduce patient discomfort. A 55 year old patient had an implant with severe bone loss in the mandibular right first molar area. Implant removal and ARP with an open-healing approach were performed using xenogeneic bone graft and collagen membrane. The implant was placed four months after ARP. The 5-year follow-up revealed that the bone and implant conditions were maintained without any complications.
