Comparison of Indices of Nonlinear Dynamics and Power Spectral Analysis of Heart Rate Variability during General Anesthesia with Enflurane or Propofol in Children

Abstract
Background: Propofol is a useful intravenous anesthetic with several benefits. However, propofol is restricted to use in children because of its side effects. Recently, regulation of the heart is evaluated with power spectral analysis or
nonlinear dynamic analysis of heart rate variability. Therefore, we compared the heart rate variability by power spectral analysis and nonlinear dynamic analysis during general anesthesia with enflurane or propofol in children.
Methods:All the patients were premedicated with atropine and lidocaine. In enflurane group (n=11, 6.2¡¾2.2 years), thiopental and vecuronium were injected and ventilated with 66% nitrous oxide and 2 vol% enflurane. In propofol group (n=12, 7.1¡¾2.0 years), propofol and vecuronium were injected and ventilated with 66% nitrous oxide and injected with propofol continuously. ECG was recorded from thiopental or propofol injection till intubation (period 1) and 20 minutes after tracheal intubation (period 2). Power spectral analysis was performed and approximate entropy, fractal dimension and prediction error were calculated from ECG data.

Results: Total frequency power, low-frequency power, approximate entropy and prediction error (p£¼0.05) were different between period 1 and 2 in the two groups. Low-frequency power in propofol group was larger than that in enflurane group in period 2 (p£¼0.05).

Conclusions: There was no difference in power spectral analysis and indices of nonlinear dynamic analysis of heart rate variability during general anesthesia with enflurane and propofol.
