Postoperative Convulsion and Respiratory Arrest Due to severe Hyponatremia in Infancy - A case report -

Abstract
Convulsion may occur in relation to hypoxemia, metabolic imbalance such as hypoglycemia and hyponatremia, fever, or occult concomittent CNS diseases in the post anesthesia care unit. Variable degree of hyponatremia commonly occur after surgery, usually as a result of intravenous administration of sodium free or hypoosmotic solutions. Aggressive management should be performed if neurologic symptoms and signs coexist. We report a case of female pediatric surgical patient who received general anesthesia for right hemicolectomy due to intussusception and had generalized convulsion following sodium chloride/dextrose 1:4 solution administration. Sodium concentration at immediate postconvulsive period revealed 121.1 mEq/§¤, sodium level was successfully restored without any sequelae after appropriate fluid and electrolyte therapy. We emphasize that it is prudent to be checked out whether postoperative hypotonic fluid is administered in the case of postoperative convulsion.
