Acute Dilutional Hyponatremia after Tonsillectomy in Children

Abstract
Severe complications following tonsillectomy in children include bleeding, aspiration or iatrogenic hyponatremia. There are still a large number of surgeons and pediatricians who immediatdy switch to hypotonic intravenous fluids in the immediate postoperative period. One of fluid problem occurring after surgery in children is acute dilutional hyponatremia. It may occur from 2 to 24 hours after surgery. In this presentation, we would like to illustrate iatrogenic hyponatremia. Postoperative hyponatremia can lead to lethal complications such as death or permanent brain damage in children. We have encountered 4 years old female patient who have developed acute dilutional hyponatrernia after tonsillectomy. Sixteen hours after surgery, headache and vomiting developed, followed by convulsion, unconsciousness. The patient was moved to intensive care unit and immediate treatment was attempted. The patient was treated sucessfully with intravenous anticonvulsant, hypertonic saline. The posttreatment course was uneventful and the patient was discharged six days after convulsion without any sequelae.
