Displacement of the Endotraacheal Tube Caused by Change of Position Pediatric tonsillectomy Patients

Abstract

|
Background: This study was done to evaluate the displacement of the endotracheal tube caused by extension of the neck and the placement of a mouth gag in pediatric tonsillectomy patients.
Methods: Forty patients, aged 5 ¡ 10 years undergoing a tonsillectomy were intubated by age-related formula size RAE-tube. Using the fiberoptic bronchoscopy, the distance from carina to tube tip was measured in neutral position and Rose¡¯s position applied mouth gag.
Results: It was statistically significant that the distance from carina to tube tip in Rose¡¯s position was longer than in neutral position (P<0.05).
Conclusions: We conclude that the position change of the neck and the placement of a mouth gag in tonsillectomy can cause to displace the tube tip and to occur accidental extubation. To prevent it, selection of proper size of RAE-tube, careful monitoring, and communication with surgeon are essential.
