Does the Emergence Speed Affect the Incidence of Emergence Agitation after Sevoflurane Anesthesia in Children?

Abstract
Background : It has been known that emergence agitation (EA) is more frequent after sevoflurane anesthesia but the etiology remains unclear. We investigated whether the EA is related to rapid emergence from sevoflurane anesthesia.

Methods : 85 pediatric patients scheduled for elective urology surgery were enrolled in the study. After induction with thiopental, atropine, rocuronium, the anesthesia was maintained with sevorflurane and 50% N_(2)O. At the end of surgery, they were randomly assigned to one of the two groups, group 1 : immediate off of sevoflurane, group 2 : gradual decrease of inhaled sevoflurane concentration with the rate of 1%/min. At PACU, EA score was recorded.

Results : No significant differences were found between the two groups in demographic data. The overall incidence of EA was 34.1%(29/85). The times from immediate off or start of decrease of sevoflurane to extubation were significantly longer in group 2. However, the incidence of EA did not show the significant differences between the two groups.

Conclusion : We controlled the speed of emergence after sevoflurane anesthesia. However, Slow emergence by gradual decrease of inhaled sevoflurane concentration did not affect the incidence of EA after sevoflurane anesthesia in children.
