Serum haloperidol level and clinical response in schizophrenic patients

Abstract
For the investigation of optimal therapeutic ranges of serum haloperidol, serum levels of haloperidol and reduced haloperidol were measured and relation between the serum levels and the symptomatic improvement was evaluated. Fixed doses of oral haloperidol were given to 22 schizophrenic patients for 3 weeks and serum haloperidol and reduced haloperidol were measured by HPLC with UV detector.
Clinical responses were evaluated by BPRS, SAPS, SANS. The results were as follow;
is No curvilinear or"linear correlation between steady..state haloperidol serum concentration and clinical response, of total subjects could be evaluated. Hbwever, in the subjects whose serum haloperidol concentration was below 30ng/ml, significant correlation between serum level and clinical improvement could be proved. In the subjects whose serum level was above 30ng/ml, a therapeutic plateau could be found.
2. Serum concentration of reduced haloperidol/haloperidol ratio was 2.84. Serum concentrations of reduced haloperidol were correlated with oral haloperidol doses.
In patients with positive symptoms, clinical response and reduced haloperidol/haloperidol ratio were negatively correlated. This finding suggests that the higher in the rate of conversion from haloperidol to reduced haloperidol, the poorer in clinical response.
In patients with negative symptoms, no significant correlation could be evaluated between clinical response and serum haloperidol concentration.
On the 10th day of haloperidol medication, serum ¢¥haloperidol concentration was positively correlated with oral dose of haloperidol.
On the 20th day, correlation between serum haloperidol concentration and oral dose of haloperidol became not significant. ..Instead, correlation between serum concentration of haloperidol and reduced haloperidol became apparent. ESP was noticed to have the tendency of relating to higher serum concentration of haloperidol.
