The Usefulness of the Balloon Dilatation in Stricture of the Parotid Ducts

Abstract
Obstruction of the parotid duct can cause recurrent parotid gland swelling. The majority of cases are due salivary calculi; however a small but important cause of obstruction is stricture of the duct. We use the guidewire and balloon systems in vascular and biliary system, GI track, too. I'd like to introduce two cases of the balloon dilatation in stricture of parotid ducts with guidewire and angioplasty balloon systems.

In the first case, the patient was 66-year-old male and had intermittent symptoms of swelling in left parotid area during 30 years. The patient has never treated previously. Recently he got the mucocelectomy by ENT surgeon. But parotid duct was dilated again. We got the his consult and determined to do intervention therapy. It was not easy to find orifice of the parotid duct per oral. We approached orifice of parotid duct by using introducer(preshaped 45 degree), 018 inch guidewire, yellow sheath(Neff percutaneous Access Set, Cook Medical, USA) with the help by ENT surgeon. We confirmed that parotid duct was dilated. We passed 035 inch guidewire(Terumo) in portion of stricture and tried balloon ductoplasty sized by 6mm¡¿40mm(ATB Advance, Cook Medical, USA). But the residual stenosis after dilatation was remained and we performed additional balloon sized by 8mm¡¿40mm. We confirmed there was no residual stenosis and parotid duct was
well reserved. In the second case, we performed same procedure. The size of balloon was 5mm¡¿40mm. The procedure was done under the conscious sedation.

We confirmed there was no residual stenosis and parotid duct was well reserved in two cases. Per oral balloon uctoplasty is a minimally invasive intervention technique. Although intervention in stricture of the parotid duct is very rarely case in our hospital, it is very useful the balloon dilatation in stricture of parotid ducts with guidewire and angioplasty balloon systems.
