Suppression of Downbeat Nystagmus with 4-Aminopyridine

Abstract

|
We determined the effect of 4-aminopyridine (4-AP) on slow phase velocity (SPV) of downbeat nystagmus in a patient with idiopathic cerebellar dysfunction. The downbeat nystagmus had a linear slow phase and increased during downward gaze, convergence, and straight head hanging. In contrast, the downbeat nystagmus was suppressed during upward gaze, and was not affected by visual fixation and lateral gazes. Ninety minutes after administration of 10 mg 4-AP, mean SPV of downbeat nystagmus decreased from 3.0 to 1.2O/s during attempted primary gaze and from 3.3 to 0.4O/s during downward gaze. As a potassium channel blocker, 4-AP appears to reduce the spontaneous upward ocular drift in downbeat nystagmus.
