Acute optic neuropathy which is induced by ischemia, inflammation, neoplasm, infection, trauma, or other variable causes can provoke a severe visual loss. Usually, the emergent optic neuropathy is accompanied by ophthalmoplegia. Fungal infection in the anterior part of cavernous sinus or orbital apex, pituitary apoplexy, and intracranial artery dissection are the representative neuro-ophthalmologic emergencies. In this section, I will discuss that acute ischemic and inflammatory optic neuropathy without ophthalmoplegia requiring an immediate treatment.
