Dermal filler injection for correction of wrinkles and other cosmetic purposes have been widely used in recent years. As the use of fillers increases, many complications also have arisen recently. Here we report a case that showed posterior ischemic optic neuropathy and ophthalmoplegia following injection of hyaluronic acid into the right forehead. A 41-year-old woman presented with right ocular pain and binocular diplopia after receiving hyaluronic acid filler injection into the both forehead for cosmetic purpose. Visual acuity of the right eye was decreased. Relative afferent pupillary defect in the right eye was presented with ophthalmoplegia and incomplete ptosis. Visual field defect on temporal field of the right eye was disclosed. Fundus photography and brain magnetic resonance imaging showed no abnormal findings. Patient was treated with high-dose steroids for 3 days followed a conservative management with complete recovery of ophthalmoplegia within 3 months.
