The Relationship between Obesity and Health-related Quality of Life in Koreans

Abstract

|
Background: We examined the relationship between obesity and health-related quality of life (HRQoL), and explored gender differences and other HRQoL related factors.
Methods: We analyzed data from the Korea National Health and Nutrition Examination Survey III (KNHANESIII). 5,526 adults, aged 19 years or older, were classifi ed as normal weight (BMI < 25 kg/m2), obesity (BMI 25-29.9 kg/m2), and evere obesity (BMI ¡Ã 30 kg/m2). HRQoL was measured by ¡°EuroQol-5 dimension [EQ-5D]¡±, which measures EQ-5D index and visual analogue scale (EQ-VAS). Logistic regression analysis was used to identify the impact of obesity on HRQoL and associated factors, while controlling for subject characteristics and co-morbidity.
Results: Men¡¯s mean EQ-5D index score (0.954 ¡¾ 0.08) was signifi cantly higher than women¡¯s (0.920 ¡¾ 0.125). Compared to normal weight group, severe obese women showed a signifi cant lower EQ-5D index score (0.914 ¡¾ 0.004 vs. 0.877 ¡¾ 0.015) after controlling for other factors, but severe obese men did not show signifi cant relationship. Severely obese women were found to have signifi cantly more problems with mobility (3.59; 1.86-6.95) and pain/discomfort (1.87; 1.12-3.15), but anxiety/depression domain of EQ-5D in severely obese men (0.25; 0.08-0.77) was better than those in normal-weight men. HRQoL by EQ-5D was signifi cantly associated with age, marital status, stress, and education level in both of sex and regular exercise only in women.
Conclusion: Obesity was associated with impaired HRQoL by EQ-5D in Korean women but not in men. More research is needed to elucidate the mechanisms which makes gender differences in the relationship with obesity and HRQoL.
