What Are the Characteristics of Obese Adults without Metabolic Complications?

Abstract
Background: Obesity is closely related to several metabolic diseases and cardiovascular complications. Recently ¡¯metabolically healthy obese¡¯ who have no metabolic complications even they are obese, were identified. We conducted
the study to find the characteristics of obese individuals without metabolic complications including body fat distribution.

Methods: Total of 291 obese adults aged 18-65 years old, who had no history of drug intake that may affect on liver function and whose body mass index more than 25 kg/m2 were selected from 2004 to 2008 among visitors at the obesity clinic in an university hospital. We analyzed self-reported questionnaires, records of physical examinations, laboratory tests, and body composition data from computed tomography. Elevated fasting glucose, hypertension, hypertriglyceridemia, and low HDL-cholesterol were evaluated as metabolic complications.

Results: Those who were obese but had no metabolic complications were 16/75 (21.3%) in men and 80/216 (37.0%) in women. After adjusting for age, daily oral intake (kcal), body mass index, smoking status, habit of exercise, family history of metabolic diseases, habit of alcohol drinking, and menopause for women, body mass index (odds ratio [OR], 0.81; 95% confidence interval [CI], 0.71 to 0.92) and HOMA-IR index (OR, 0.39; 95% CI, 0.20 to 0.76) were significant factor for obesity without metabolic complications in women. Homeostasis model assessment for insulin resistance (HOMA-IR) index was marginally significant in men (OR, 0.20; 95% CI, 0.04 to 1.04).

Conclusion: Lower insulin resistance and lower body mass index were associated with obesity without metabolic diseases among obese women.
