Management of Dyslipidemia in Primary Care

Abstract

|
Cardiovascular disease (CVD; coronary artery disease, cerebrovascular disease, and peripheral arterial disease) is a major cause of morbidity and mortality. Dyslipidemia is a significant predictor of CVD in high-risk patients, such as type 2 diabetics. Even though statin drugs have proved effective in lowering CVD risk, in patients at moderate or high risk for CVD, therapeutic lifestyle change should be emphasized as an important component of therapy regardless of plasma low density lipoptotein (LDL) cholesterol level. The primary therapeutic target is LDL cholesterol and should be treated fi rst with a statin. Even with adequate LDL cholesterol lowering, many patients on statin therapy have signifi cant residual CVD risk. Ezetimibe, fi brates, niacin and n-3 fatty acid ethyl esters are potentially useful treatments for residual risk of CVD due to uncorrected dyslipidemia (elevated plasma triglycerides and low high density lipoprotein cholesterol) in statin-treated patients.
