Interpretation of Liver Function Test

Abstract
Evaluation of liver function can often be achieved by a test profi le known as a liver function test, many components of which are not unique to the liver but, when evaluated together, allow for accurate diagnosis of abnormalities of liver function. A rational approach for the appropriate evaluation of serum liver chemistry is essential for providing high-quality, cost-effective health care, although there are no controlled clinical trials examining the optimal approach for evaluating serum liver chemistry. It is important to emphasize that the initial steps in interpreting abnormal liver tests includes a detailed history, inventory of medications, and a physical examination. This should include an assessment of the patient¡¯s risk factors for liver disease, alcohol consumption, comorbid conditions, and signs and symptoms of hepatobilliary diseases. A critical step in guiding the evaluation is determining the overall pattern of the abnormal liver function tests, which can be broadly divided into three categories: aminotransferase elevation, bilirubin elevation, and alkaline phosphatase elevation. The diagnosis of specifi c liver diseases can be made from specifi c patterns of the liver function test and from specifi c viral antigen, antibody, and immunologic assays, as well as radiologic fi ndings.
