A Case of Fulminant Type 1 Diabetes

Abstract
Fulminant type 1 diabetes was fi rst proposed as a distinct subtype of idiopathic type 1 diabetes in 2000, characterized by abrupt onset of hyperglycemic symptoms, diabetic ketoacidosis, the absence of diabetes-related autoimmune features, and elevated serum pancreatic enzyme concentrations. In a nationwide survey in Japan, 19.4% of acute-onset type 1 diabetes was diagnosed as fulminant type 1 diabetes. In Korea, 7.1% of type 1 diabetes belongs to fulminant type 1 diabetes. There are many case reports of fulminant type 1 diabetes from East Asian populations, while just a few cases were reported from Caucasian populations. Fulminant type 1 diabetes is a notable subtype of type 1 diabetes in East Asia. There are three suggested pathophysiologic mechanisms: genetic factors, viral infection, and accelerated immune reaction; however, these mechanisms are still uncertain. We present in this study a case of fulminant type 1 diabetes with a typical clinical course. We report this case with review of the literature.
