Usefulness of Waist Circumference for Evaluation of Insulin Resistance in Postmenopausal Women

Abstract
Background: Insulin resistance is a risk factor for diabetes and atherosclerotic diseases for postmenopausal women, and metabolic syndrome reflects insulin resistance. Waist circumference (WC) is the most easily registered component of metabolic syndrome. The objective was to compare WC alone with metabolic syndrome as defined by the National Cholesterol Education Program Adult Treatment Panel III (NCEP-ATP III) guideline for their usefulness in diagnosing insulin resistance in postmenopausal women.

Methods: This is a cross-sectional study by 225 patients who visited a Health Promotion Center of Kyeonggi-do from January 2 to December 31, 2011. We analyzed the association of WC, systolic blood pressure, insulin resistance (homeostatic model assessment-insulin resistance), and plasma concentrations of fasting glucose, high density lipoprotein cholesterol, and triglyceride adjusted for age. We evaluated the optimal cut off point of WC that refl ects insulin resistance. The ability to identify insulin resistance correctly by means of metabolic syndrome and WC was measured as area under the receiver operator characteristic (ROC) curve, sensitivity, specifi city, positive predictive value, negative predictive value, and accuracy.

Results: There was a strong positive association between WC and insulin resistance. According to the ROC curve, the optimal cut-off point for WC predicting insulin resistance was >83 cm. Using the NCEP-ATP III guideline, WC>83 cm alone is nearly as useful as metabolic syndrome in diagnosis of insulin resistance in postmenopausal women.

Conclusion: WC>83 cm in postmenopausal women indicates a high likelihood of insulin resistance and is almost as useful as the NCEP-ATP III guideline in diagnosis of insulin resistance.
