Treatment for Osteoporosis: Focusing on Bisphosphonate Therapy

Abstract

|
|
The primary goal of treatment for osteoporosis is prevention of osteoporotic fracture. International treatment guidelines recommend the use of bisphosphonates as fi rst-line therapy in patients with osteoporosis. However, anti-fracture effiacy of bisphosphonates is largely dependent on patient-related factors such as compliance and persistence. Selection of a specific bisphosphonate should be based on a review of anti-fracture efficacy, fracture risk of the patient, adverse events and safety, convenience of administration, cost-effectiveness, and the patient’s preferences. Most importantly, risks and benefits of bisphosphonates should always be presented to the patient. This review describes the structure, pharmacokinetics and mode of action of bisphosphonates, anti-fracture efficacy, and common clinical issues with bisphosphonate treatment in clinical practice. Also, controversies and safety issues surrounding their use are discussed.
