Treatment for Osteoporosis: Focusing on Bisphosphonate Therapy

Abstract

|
The primary goal of treatment for osteoporosis is prevention of osteoporotic fracture. International treatment guidelines recommend the use of bisphosphonates as fi rst-line therapy in patients with osteoporosis. However, anti-fracture effiacy of bisphosphonates is largely dependent on patient-related factors such as compliance and persistence. Selection of a specific bisphosphonate should be based on a review of anti-fracture efficacy, fracture risk of the patient, adverse events and safety, convenience of administration, cost-effectiveness, and the patient¡¯s preferences. Most importantly, risks and benefits of bisphosphonates should always be presented to the patient. This review describes the structure, pharmacokinetics and mode of action of bisphosphonates, anti-fracture efficacy, and common clinical issues with bisphosphonate treatment in clinical practice. Also, controversies and safety issues surrounding their use are discussed.
