A Prevalence of Anxiety and Depression for an Elderly Diabetic Patients and the Relationship of Quality of Life

Abstract
Background: This study was designed to evaluate the prevalence of depression and anxiety disorder among diabetic outpatients over 65 years old by using the Korean version of the Hospital Anxiety and Depression Scale (HADS) and how those diseases affect quality of life.


Methods: Subjects included male patients over 65 years old with type 2 diabetes who visited family medicine or internal medicine department of Daejeon Veterans Hospital in Daejeon from March to May, 2012. Depression or anxiety were measured by using HADS. Quality of life (QOL) was measured by using World Health Organization Quality of Life Assessment-Brief (WHOQOL-BREF).

Results: There were 38 depressed diabetic groups (43.7%). Glomerular fi ltration rate (58.0¡¾19.3 vs. 67.0¡¾20.2), triglyceride (122.5¡¾61.4 vs. 171.5¡¾116.2) level, and income (31 [81.6%]) in depressed patients were significantly lower than ondepressed patients. There were 26 anxious diabetic patients (29.9%). In anxious patients, compared with non-anxious
patients, a higher cormobidity index was observed (2.0¡¾1.1 vs. 1.2¡¾1.0), non-alcoholics (23 [88.5%]), and a longer uration of diabetes (18 [69.2%]). The physical health, psychological health, social relationship, environmental domain, satisfaction of QOL, health satisfaction and WHOQOL-BREF total scores were found signifi cantly lower in depressed patients and anxious patients than non-depressed patients and non-anxious patients.

Conclusion: Elderly diabetic patients are likely to develop depression and anxiety, and this results in poor quality of life.
This is not associated with current glycemic control status or the development of diabetic complications but with social factor like duration of diabetes, cormobidity, income, and drinking.
