Vitamin D Deficiency in Obese Male Children and Its Association with Insulin Resistance and Adipokines

Abstract
Background: Recent studies have reported that vitamin D deficiency may be involved in childhood obesity and insulin resistance, as well as adipokine dysregulation. Th erefore, we investigated the association of serum vitamin D levels with insulin resistance and adipokines in Korean male children.

Methods: Forty-one obese and 30 lean boys, aged 12 to 13 years, were participated in this study. In all subjects, body mass index (BMI) and blood pressure (BP) were measured. Fasting glucose, insulin, 25-hydroxyvitamin D (25[OH]D), aspartate aminotransferase (AST), alanine aminotransferase (ALT), total cholesterol, leptin, and adiponectin levels were also determined. Insulin resistance was assessed by homeostasis model assessment for insulin resistance (HOMA-IR), homeostasis model assessment for insulin sensitivity (HOMA-S), and quantitative insulin sensitivity check index (QUICKI).

Results: 25(OH)D, HOMA-S, QUICKI and adiponectin were signifi cantly lower in obese children than in controls. However, BMI, BP, HOMA-IR, AST, ALT and leptin were signifi cantly higher in obese compared with lean subjects. 25(OH)D was negatively related to BMI, fasting glucose, insulin, HOMA-IR and leptin, while positively related to HOMA-S and QUICKI in all subjects. In a multiple regression analysis, after adjustment for age and BMI, fasting glucose showed the greatest explanatory power to the 25(OH)D.

Conclusion: We suggest that vitamin D may play an important role in insulin resistance and weight gain in obese male children by regulation of leptin. Further studies are needed to elucidate a pathogenic link between vitamin D defi ciency and obesity including the modulation of adipokines.
